Position Overview & Specifications

About The Role

Location: Hybrid role covering Bolsover & North East Derbyshire – South Derbyshire – South Derbyshire and Derbyshire Dales, with homeworking.
Contract: Part-time Fixed Term Contract until March 2024, with possibility of extension.

We have an exciting new opportunity for a passionate and compassionate individual to join us as a Dementia Adviser in our Derbyshire team.
The Dementia Adviser's role involves dedicated members of staff who provide 1-1 support over the phone or on a visit to a persons’ home. This support may include care navigation, information and advice for people diagnosed with dementia their carer’s and those who are worried about their memory loss, who may not have been formally diagnosed but who need support in obtaining a diagnosis. While also helping people to understand dementia, cope with day to day living and preparing for the future.

You will also assist service users to access support and help from others, helping them to navigate other services, providing signposting and referrals, as well as facilitating support groups and other group activities. Working with partners and key agencies is a crucial element of this role e.g., GPs, other services and sectors that support people with dementia.

About you

  • We are looking for individuals who have an understanding of dementia and the needs of those living with dementia and their Carers.
  • Your ability to assess client needs along with the understanding of the need for client confidentially is essential to providing a valuable and worthwhile service.
  • To be successful in this role you will have excellent non-judgmental, communication skills as well as strong organisational and time management skills.
  • Good IT skills and experience working with databases is also essential.
