Hospital Discharge Assistant
Main area: Medicine, YYF
Grade: Band 4
Contract: Permanent
Hours : Full Time 37.5 hours per week to include some unsociable hours

An exciting opportunity has arisen to join the Hospital Discharge Team within Medicine based in Ysbyty Ystrad Fawr, Caerphilly. The successful candidate will work closely with patients, relatives, medical staff, social services, third sector and wider support services on a quality improvement initiative to support safe and timely ward discharge processes.
The successful candidate will need to have good knowledge of current infrastructures and processes within the acute hospitals as well as the mechanisms in the community to facilitate a safe discharge. Training and an induction will be offered to the successful candidates who will be responsible to the Hospital Manager, YYF.
The candidate will need to demonstrate an enthusiasm for patient flow as well as service improvement, good skills in excel, and an ability to work independently as well as part of the ward team. Good communication skills are essential to this role as are being creative, adaptable and tenacious whilst working within the boundaries of the role and forging therapeutic working relationships with the ward staff and wider multi-disciplinary team.
The hours of duty will be 8:00 – 16:00.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
