Position Overview & Specifications

We are looking for a highly motivated Band 4 Mental Health Rehab Assistant to join an exciting new team being launched in Kensington & Chelsea and Westminster (KCW).

The purpose of the Transitional Team is to support the mental health inpatient rehabilitation service and patients who are placed out of borough to optimise the likelihood of patients being able to reside in the community. The post holder will work under the direction of a Band 7 Mental Health Professional and link in with the relevant system partners to identify and work on barriers to move on as well as provide continuity of input and support during transition back into the community. Service users will be supported to build and maintain positive relationships with people around them, including their family, friends as well as their named worker. This In turn will improve patient experience and supporting flow in the wider system.

The team will consist of two Band 7 Rehab Transitions Leads and two Band 4 Rehab Assitants covering KCW. Each Band 4 will be allied with one of the boroughs i.e. one with Westminster and one with Kensington and Chelsea, but will also co-operate and work closely with their counterpart depending on service demands. The team will be supported by the Occupational Therapist Lead across KCW.

We are looking for someone who is dynamic and motivated and has the clinical skills and experience to make this exciting new project a success.

This will be a challenging and stimulating role for someone who is great at building relationships with our service users and motivating and supporting them to link in with their local community resources. As well to provide some continuity and support to help service users bridge the transition from an inpatient mental health rehab setting back to their community.

The post holder will be self motivated and energetic as well as being able to work with the direction of their Band 7 lead.

Relationships across multiple services will be key to this role, including with the Community Mental Hubs, the in-Trust Rehabilitation services and external providers.

The post holder will work closely with people who access services, their carers and their wider support network colleagues, commissioners and care providers to inform and contribute to achieve the best outcomes for all.

CNWL is strongly committed to staff development. There will be extensive training opportunities on offer with a range of options tailored to both service need and the individual’s interests.

Effective discharge planning and early identification of potential discharge barriers is key, meaning an ability to problem and solve and identify those barriers proactively, taking a lead where appropriate.

The post holder will work closely with local Community Teams to facilitate a safe and smooth transition to the community.

The team members will expected to be mobile and flexible in response to service need when working with service users who are outside of borough or in rehab placements, including outside of London, primary in Surrey.
