Position Overview & Specifications
You will work to support the Physiotherapy, Occupational Therapy and Nursing needs of patients in Milton Keynes, helping to provide holistic physical health treatment plans in community and inpatient rehabilitation settings. This will assist in preventing our patients being admitted to acute services and support their discharge from hospital. This community-based role will require use of your empathy, communication and partnership working skills. You will have the opportunity to have supervised support from the Home 1sttherapists and work in co-operation with the wider multi-disciplinary team in planning patient’s discharges to maintain patient flow.

It is essential to be a car driver and have access to a car for use at work.
- To undertake specific support work to aid rehabilitation of service users, in the achievement of optimal functional independence, with or without the assistance of others.
- To provide flexible programmes and interventions of therapeutic activities, as directed by Home 1stAllied Health Professionals (AHP) or Band 4 Rehabilitation Assistants.
- To work without direct supervision, monitoring progress and minor adaptations to interventions as required, under the guidance of Allied Health Professionals or Band 4 Rehabilitation Assistants.
- To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home1st, as well as outside agencies.
- Work within any of the Home 1stTherapy pathways and participate in rotation to different therapy teams as required.
- Participate in development of integrated pathways.
- To be responsible for the ordering and provision of Allied Health Professional or Band 4 Rehabilitation Assistant prescribed equipment.
