Position Overview & Specifications

An exciting opportunity has arisen for dynamic, motivated and innovative pharmacists to join our expanding Medicines Management team at Central London Community Healthcare Trust.

We are looking for part time and full time pharmacists to provide a clinical pharmacy service to our inpatients in our Rehabilitation Service across the Trust.

As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients ensuring an integrated approach to the management of medicines.

If you are successful you will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties. You will be supported by experienced Senior Clinical Pharmacists allowing you to gain new skills in each rotation and the ideal opportunity to develop strategic, quality improvement and leadership skills.

The post holder may be required to travel across the different sites within London where we have Bedded Services inpatients so will require some flexibility.

Central London Community Healthcare (CLCH) is one of the largest community healthcare organisations in London and Hertfordshire, providing our services to diverse communities/boroughs in 11 London Boroughs - Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hounslow, Kensington and Chelsea, Merton, Richmond, Wandsworth, Westminster - and Hertfordshire.
  • To provide and develop a high quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Bedded Services Pharmacist.
  • To provide support and guidance to Medicine Management Pharmacy Technicians
  • To be responsible for individualised patient pharmaceutical care ensuring clinical prioritisation on a day to day basis at ward level.
  • To provide a patient focused pharmacy service to patients and staff including medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized e.g. NICE, NHSE/I, PHE
  • Facilitate changes to the patients’ own use of medicines to improve safety, adherence and empower as much independent use of medicines as possible and appropriate.
  • To support and implement efficient medicines management initiatives to reduce drug waste and drug spend and to support patient flow and facilitate timely discharges
  • To participate in ward rounds and MDTs where appropriate to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings.
  • To optimise medications by undertaking Structured Medication Reviews and provide counselling to all patients
  • To participate in collecting and recording activity and intervention data to monitor key performance indicators.
  • To participate in ADR and Datix reporting and implement risk reduction initiatives at ward level
  • To provide services with an integrated approach by developing and maintaining excellent working relationships with multidisciplinary teams within CLCH and local organisations (CCGs, PCNs, GPs, Acute Hospitals, Care Homes, Community Pharmacies) to deliver optimal and seamless patient care by providing clear and effective communication of patient’s pharmaceutical needs in a timely manner
  • To support the implementation of the Medicines Management Strategy in linewith national, regional and local policy
  • To attend and contribute to team meetings on a regular basis
  • To support and undertake the management of projects and service developments to deliver the required aims and objectives
  • To be a Link Pharmacist for a variety of audits / projects
  • To assist in service development and change management required for the development of the Bedded Services team in providing an efficient and optimised Clinical Pharmacy Service at ward level.
