Position Overview & Specifications

Wigan Community Mental Health Services are an integrated health and social care service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties.

  • To manage a defined community caseload within a Community Mental Health Team (CMHT) using client centred and evidence based practice to assess, plan, implement and evaluate interventions, under the supervision of a nominated Band 7 case manager.
  • To act as care coordinator for a specific client group that in the majority are noncomplex
  • You will have a role in the organisation and delivery of depot injection and will also support the Clozapine clinics
  • You will be required to undertake as lead professional, systematic reviews of commissioned packages of care for service users who have been placed on the review pathway within the CMHT, ensuring placements remain appropriate and offer the best value.
  • To receive regular clinical and managerial supervision.
  • You will be support with the duty role of the CMHT.
This is an exciting opportunity for an enthusiastic Registered Mental Health nurse to join our friendly Community Mental Health Team based across Claire House (Wigan) and Pennington Unit (Leigh)

This post provides a challenging opportunity to work within an established fully integrated and supportive team.

You will hold and manage a caseload of service users who require care coordination under the Care Programme Approach. By assessing your clients and carers needs you will be able to access and sign post to a wide range of community resources and provider services available in Wigan and Leigh.

You will receive regular clinical and managerial supervision and developmental appraisal thus enhancing your current practice and future career prospects.
