We are currently recruiting three Information Assessment Officers that will be based in Adult Services at Hackney. You will be responsible for receiving referrals by ‘phone, email and letter, entering accurate information and case recording onto the Mosaic system, completing a range of assessments including social care, telecare and Trusted Assessor assessments to provide simple equipment for older and disabled adults. You will also provide information, advice and signposting to residents and their families/friends/carers, combined with receiving and raising safeguarding concerns and bringing these to the attention of the duty Senior Practitioner.
You will be able to demonstrate knowledge of Adult Social Care and evidence your experience of working in an Adult Social Care service.
