What would you be doing at HomeLink?*

• Provide person centred Physiotherapy care and support to adult patients, their families and carers in their usual place of residence.
• Manage patients referred to HomeLink Healthcare for assessment, clinical diagnosis, treatment plan and implementation, and evaluation of care delivered.
• Perform assessment of patients with diverse and complex conditions.
• Work flexibly across the community as required.
• Participate as part of the multidisciplinary team.

What would we expect from you?

• HCPC registration with a minimum, of 3 years experience.
• Right to work in the UK without any form of sponsorship.
• You must have a drivers licence and access to a car, this is essential as its a mobile role.
