Position Overview & Specifications

Community Nursery Nurse (Band 4) Health Visiting Team –Hounslow

Due to changes in service delivery, there are fantastic opportunities for enthusiastic and innovative Community Nursery Nurses to join our friendly, supportive 0-19 team in West London.

We work with families in one of the most diverse boroughs in London to give children the best possible start in life by providing the most up-to-date evidence based support.

As part of our team, you will contribute to the delivery of the national 0-5 years Healthy Child programme across Hounslow, working closely with partners, such as such as GPs, children’s centres and social care, providing seamless care to the population of Hounslow. You’ll also have an important role helping to recognise and protect vulnerable children.

JOB PURPOSE-The community Nursery Nurse works as part of the corporate health visiting team supporting the delivery of the health child programme to a defined population including well baby clinics and completing health reviews. The community Nursery Nurse will also offer packages of care to families in the community as directed by the Health Visitor. The work is offered in a variety of community settings, which include clinic, home, early year’s settings & children’s centres.

KEY RESPONSIBILITIES
  • To agree a planned programme of care in partnership with the parent /carer and Health Visitor and implement this virtually or in the home or clinic setting.
  • To work within the skill mix Health Visiting team on own initiative, and prioritising workload
  • To communicate with the Health Visiting Team to ensure all health promotion public display notice boards.
  • Knowledge of infant feeding and nutrition and the ability to provide guidance and support
  • To maintain contact as appropriate with local nurseries to promote health lifestyles.
