Position Overview & Specifications

We are looking for a highly motivated, enthusiastic dietetic associate practitioner with an, interest in the Learning Disability Services

You will work with the Dietitians to provide dietetic support and advice to service users and carers in relation to weight management, nutrition support, diabetes and enteral feeding.

The Community Learning Disability team provides multi professional health services for clients aged 18+ who have a learning disability. Led by the Clinical Team Manager, team members include; Dietitians, Nurses, Occupational Therapists, Psychologists, Psychiatrists, Physiotherapists, Speech and Language Therapists and Health Support Workers.

An ability to travel throughout the Leeds area to meet the demands of the post is essential. We will offer you the opportunity to develop your skills through supervision and a continuing professional development practice.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. You will be a valued member of our highly supportive multi-disciplinary, community learning disability service and AHP community. You will manage an identified caseload of clients with a range of clinical need including weight management, nutrition support, diabetes and enteral tube feeding (some of which will be complex). You will provide a range of dietetic support and interventions to adults with a range of complex health needs and a learning disability working closely with a range of health and social care partners (including the private and voluntary sectors and adult social care).

To manage own caseload delegated by and responsible to the registered dietitian, working without direct supervision, within their scope of practice in a variety of community settings including the service user’s own home.

To provide a range of dietetic support and interventions to adults with a range of complex health needs and a learning disability.

To provide dietetic support to staff/carers to help them implement dietary advice for the service users who require help to achieve identified goals and promote independence and wellbeing

To promote the role of nutrition in health and disease prevention

To promote a positive image of service users with complex health needs and a Learning Disabilities at all times

To promote a positive image of the dietetic and community services of Leeds and York Partnerships NHS Foundation Trust at all times

You will be expected to support and develop dietetic provision within community services. Working in partnership with other organisations.
