Job Hours: 37 Hours / 37 Awr

This is a newly formed team in Merthyr Couty Borough Council that has been established to meet the requirements of the Discharge to Assess policy and to provide effective, seamless support to adults who are being discharged from hospital. This will involve supporting the person for up to 8 weeks post discharge to ensure there is an effective plan in place that reduces the risk of re-admission to hospital.

You will need to complete parts 1 and 2 of the course and must be able to evidence this prior to starting work with the Authority. This post will be responsible for the assessment of people in hospital/being discharged, identifying the package of support required to move them back into the community safely, developing the plan to support this and reviewing whether the plan is being effective in relation to prevention of re-admission.
