Position Overview & Specifications

The post holder will be required to work on their own initiative and demonstrate a willingness to help others and be a key member of the team, with responsibility for specifically allocated tasks.
The post holder will be expected to follow set processes, prioritise their workload and demonstrate initiative in order to fulfil the requirements of the post. Excellent interpersonal and communication skills are essential plus an ability to deal with sensitive and confidential phone calls. The post holder will be required to be proficient in word processing and inputting into databases, in addition to diary management.

To support the brokerage of care for those CHC patients who have been deemed eligible individuals, following guidance on clinical needs, and in doing so deliver appropriately commissioned care placements and packages of care.

  • To support the brokerage of care for those CHC patients who have been deemed eligible individuals, following guidance on clinical needs, and in doing so deliver appropriately commissioned care placements and packages of care.
  • Work across all service user groups eligible for CHC and Children’s Continuing Care, including older people, disabled adults, people with learning difficulties, people with mental health needs, children and young people and ABI/TBI.
  • Communicate effectively with care providers to identify suitable CHC care packages and placements.
  • Update systems to provide oversight of available care market capacity.

Issue provider contracts and individual placement agreements for CHC funded placements and care packages
  • Engage compassionately and professionally with service users, families and representatives when identifying care packages and placements.
  • Act promptly to address identified risks in care provision, or where changing needs require alterations to the way that care is delivered.
  • Maintain accurate information systems to evidence the effectiveness of the service
  • Develop good links with CHC operational teams, acute hospitals, and the three local authorities.
  • Deliver against personal objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.
  • Propose changes to own work, or that of the wider team, to achieve a more effective service.
  • Ensure all funding is signed and in place prior to placement
  • To receive requests for a care service of the placement requirement from initial request for a service, identifying the most appropriate Provider and placing with the service.
  • Where services are necessary – work with CHC assessors, case managers and external providers to help source, negotiate and broker the service for service users based on clinical need, outcome, cost and efficiency.
  • Liaise with senior staff to ensure complex cases are escalated in a timely manner so as to ensure that the service user receives care from the most appropriate Provider
