Role Overview & Responsibilities

An exciting opportunity has arisen to support the continued development of Neighbourhood Working in Lincolnshire. This role will work in East Lindsey Neighbourhood in partnership with East Lindsey and Meridian Medical Primary Care Networks.

Neighbourhood Working is an integrated, collaborative approach which brings stakeholders together from across the health, care and community/voluntary system at local population level. Working with some of the most vulnerable and high intensity users of health services; neighbourhood working puts people's health and wellbeing needs at the centre of decision-making and local solution finding, overcoming barriers and promoting timely and proactive support so people stay independent and close to home.

Neighbourhood Coordinators provide a crucial link between all stakeholders contributing to neighbourhood working, acting as a front door into neighbourhood team, receiving referrals and targeted data lists, gathering clinical information and maintaining systems to track and evaluate neighbourhood working activity.

Coordinators build relationships, develop and share knowledge and expertise to facilitate collaboration. They coordinate regular and bespoke multi-disciplinary meetings, tracking and taking actions in support of care planning and case management. They also coordinate, forums for collaboration and learning and supporting neighbourhood hubs.

The post is offered as a 6 month fixed term contract or secondment offering an excellent opportunity to develop and build skills and relationships in a supportive and progress focused team. The post is for 22.5 hrs - 3 days a week Weds, Thursday, Friday.

Please note that the base for this role will be Louth Hospital. The successful postholder will work from the Louth neighbourhood working hub and play a pivotal role in engaging with stakeholders working across the East Lindsey area. The post holder may be required to travel to meetings and events locally.

The team are equipped to work flexibly from a variety of bases across the neighbourhood and enabled to work from home from time to time.
