We are looking to recruit a full-time/Part-time permanent GP Community Link Workers
Part-time positions will be pro rata
This is an exciting opportunity to work as part of a Primary Care Multi-disciplinary team. The CLWs will provide a person-centred approach in response to the needs and interests of GP practice patients supporting them to identify issues that affect their ability to live well. The CLWs will work collaboratively with patients supporting them through decision-making processes to identify issues that they would like to address and help them to overcome barriers to improving these.
Please note: The postholder will have
- UK/EU/EEA Driving Licence access to a vehicle and a willingness to travel within the local area.
- Ability to work flexible hours as early evening work may be required at times.
- Ability to work within the homes of patients on some occasions. Shift pattern Monday-Friday 0900-1700, days and times to be confirmed
KNOWLEDGE, TRAINING, AND/OR EXPERIENCE REQUIRED TO DO THE JOB.
- Educated to HNC level or above in a health or social care-related subject or knowledge acquired through demonstrable experience
- Experience working with vulnerable adults.
- Health improvement knowledge and practice
- Highly effective interpersonal and communication skills in working with people on a 1:1 basis and within organisations.
- Experience working in a community, hospital or social care setting.
- Be able to work independently and within a team liaising with colleagues as required.
