Position Overview & Specifications
The Home First Reablement Unit is a 30- bedded unit offering multi-disciplinary assessment, rehabilitation and reablement to patients recovering from acute illness to promote functional recovery and independence before discharge. The Physiotherapy and Nursing teams work collaboratively to support the patients recovery and this role would enhance the exercise therapy provision to our patients.
The post holder will provide tailored exercise support for patients to achieve their reablement goals and recovery their independence with ADLs. We are looking for two enthusiastic exercise professionals with experience in exercise prescription and delivery.
The Exercises Professional will work as an active member of the Physiotherapy team and aims to support patients with functional and physical deconditioning. Your caseload will be identified and allocated by the physiotherapy team, and you will work both independently and with supervision to deliver individualised and group exercise.
The Exercise Professionals will have responsibility for their own case load with advice and support from the Physiotherapy staff.

- To manage an allocated workload efficiently, and effectively with regard to clinical priorities and use of time, in accordance with agreed guidance/care pathways.
- Collect relevant patient information from a variety of sources to assist with patient management e.g. ward handovers, observation charts, medical, nursing records and Physiotherapy notes.
- Use good communication to gain valid informed consent prior to undertaking any clinical tasks. With support, to work within the legal framework with patients who lack the capacity to consent to treatment.
- Use a range of verbal and non-verbal communication skills to communicate effectively with patients, carers and colleagues.
- Use skills of motivation, explanation, and empathetic listening to encourage patients to undertake their treatment programme and optimise rehabilitation potential and/or quality of life. Barriers to effective communication may regularly be evident, e.g. learning difficulties, pain, psychological/emotional status, altered perception, dysphasia.
- Communicate responsibly and effectively with patients and carers. To use information gathered in a professional manner ensuring patient confidentiality is in line with the Trust Code of Conduct.
