Position Overview & Specifications
We are looking to recruit two motivated and enthusiastic Community Support Workers to join this exciting endeavour. The successful candidates will have experience working with people with mental health needs across a range of services.
You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.
The team is a trust wide service covering all five boroughs across the Pennine Care footprint. We are currently based at Trust HQ in Ashton Under Lyne although this is subject to change. Our core hours are Monday to Friday 9am-5pm.
To work as part of the multidisciplinary team and provide support on both an individual and group basis to individuals experiencing complex psychosis.
To support service users to achieve their full potential through accessing resources within their communities and equipping them with the skills and confidence to enable them to manage their lives independently.
The role will involve delivery of therapeutic recovery focused and community activities, and individualised treatment plans under the guidance of practitioners within the team.

To provide support to an allocated group of service users, promote their recovery and help to maintain them in their community environment.
To assist qualified practitioners in assessment, planning, implementing and evaluating individual care plans.
To ensure that service users engage effectively with the agreed care plan and access appropriate services.
Positively promote independent living of service users within the community.
Provide support and interventions to service users in their own homes and the wider community based upon their assessed needs.
Assist service users to make full use of community resources.
