Position Overview & Specifications

An exciting opportunity has arisen for a Band 6 Community Case Manager within the Home Based Intermediate Care team, who is passionate about community nursing, rehabilitation as well as providing nursing support to people within their own homes and urgent care pathways. You will join the Integrated Care Service (iCares) which works across Sandwell Community providing rehabilitation and admission avoidance in a variety of settings.

Due the recent expansion and new investment in the Home Based Intermediate Care Team, there is the opportunity to bring your leadership and clinical skills to help guide our evolving service, particularly in light of the introduction of the Discharge To Assess initiative that has been implemented nationally and focus on a ‘Home First’ approach.

Within the Home Based Intermediate Care Team we have Occupational Therapists, Physiotherapists, Community Matrons, Assistant Practitioners and Rehab Support Workers, delivering integrated care and supporting people in their own homes.

The posts sit within the iCares Directorate which is expanding due to investment. The community nurses are integral to integrated pathways and improving health outcomes for our population.

The key duties for the post holder will be;

The post holder will have continuing responsibility for proactively monitoring and managing patients on a caseload within a designated pathway.

You will be expected to recognise the early symptoms of disease exacerbation, acute illness and injuries, as well as social and familial factors based on the understanding of long term conditions, the effects on the individual, their family and environment as well as current evidence and practice standards.

To provide leadership and oversight of the people on the caseload.

You will work closely with your peers, a Band 7 lead, the Service Lead and other colleagues across the health and social care system.

To deliver and evaluate the care provided to patients on a daily basis.

To provide specialist assessment and treatment of patients with planned and unplanned presentations within community settings.

To supervise, assess and educate team members including bank or agency staff, support workers and students.

We would be happy to support experienced Band 5 nurses who still need some structured support to make that leap to their first Band 6 post.
