Position Overview & Specifications
As a Mental Health Liaison Practitioner in our service you will be working as part of a Multi-disciplinary team which include psychologists, nurses, occupational therapists and medics.

You will work with people with mental health problems, providing specialist short term interventions with a focus on recovery and reablement .You will act in the role of Lead Practitioner for a defined caseload of clients. We encourage knowledge sharing and teamwork so all team members are involved with the full range of best practice interventions. You will benefit from regular caseload reviews and we are currently developing family interventions within our service, with plenty of opportunities for you to become involved.
Requirements
- A Registered Mental Health Nurse or Occupational Therapist
- Experienced in working with people with mental health problems that would benefit from the clinical interventions and expertise provided by secondary mental health services.
- A team player who is willing to collaborate and strive for best practice
- Experience within a community assessment and treatment service

To work in partnership with GPs and other primary care colleagues around individual patients and to work jointly with them to develop care plans for the client.
To undertake routine assessments which have been triaged to the role.
To maintain a close working relationship with local secondary care teams, keeping them informed of developments within primary care.
To build professional relationships within Primary Care Networks to support effective development of care pathways within localities.
To work closely with secondary care colleagues, sharing expertise where necessary to ensure a consistent MDT response.
To have an awareness of available local resources and services and how to access these
