Role Overview & Responsibilities
An exciting and unique opportunity has arisen for a dynamic healthcare support worker to help facilitate Physical Health Monitoring clinics across Swansea Bay Adult Community Mental Health Teams. The post will ensure we are delivering a high standard of physical health monitoring in line with both NICE and local prescribing guidelines such as ADHD and antipsychotic monitoring, some of which are already set up. The post holder will work alongside a registered nurse and input into clinics to ensure smooth processes and work closely with all Team Managers within the community mental health team.
We are looking for a nurse with excellent knowledge of both mental health care and the services we provide but also experience in providing physical health monitoring to our client group or looking to expand their knowledge in this field.
Travelling between sites is essential for this role as it will involve input to 5 Community Mental Health Teams across Swansea Bay.
