Position Overview & Specifications

Emergency Department 60:40% Clinical Fellow Post (ST4+) – Paediatric Blast Injury

Founded in 2017 this is a collaboration between Save the Children UK, Imperial College London, the World Innovation Summit for Health, Bristol Royal Infirmary and Bristol Royal Hospital for Children emergency departments. This was in response to the global crisis that places millions of children worldwide at daily risk of conflict injury and the lack of paediatric specific experience and training in these areas. In 2018 the Partnership released the Paediatric Blast Injury Field Manual, a clinical resource for first responders and clinicians dealing with injured children in conflict zones.
Successful applicants will be appointed to a 60/40 clinical post at Bristol Royal Infirmary with 40% funded time to work on any of the following projects.

1. Update of the Paediatric Blast Injury Field Manual and preparation of material for digital conversion to a web based app to allow great worldwide access
2. Creation of a field pocket action card and haemorrhage control pack for first responders dealing with injured children
3. Non-pharmacological paediatric pain management research project with an aim to produce clinical resource material for care providers
4. Development of a remote paediatric conflict trauma training package for Save the Children in Afghanistan

We would also consider supporting any related project that a candidate wishes to propose.

The post will be a combination of service provision including out of hours and paid time to develop a specialist interest. The candidate to combine a job in Emergency Medicine with a 40% time allowance for a specialist interest. Specialist interests may include but are not limited to education (SIM/FOAMed), global health, expedition medicine, leadership and management, advanced airway management, research (with Professor Jonathon Benger), ultrasound, resuscitation, toxicology and paediatrics. If an applicant has a particular specialist interest they would like to pursue not listed above then we will accommodate. Open to trainees and non-trainees. For those trainees who already have a UK National Training Number in Emergency Medicine, these posts are eligible for OOPT or OOPE.

Clinical

The reception, diagnosis and emergency treatment of patients presenting to the adult Emergency Department. The successful candidate will be given supervision and support from senior medical staff to develop competency in the autonomous clinical management of these patients whilst ensuring patient safety. This post is based in the adult Emergency Department and no sessions will be undertaken in the paediatric Emergency Department.

There is close liaison between ED and ICU at BRI. Patients referred to Specialist Units within the hospital are the responsibility of that specialist team, who will see the patient in the Emergency Department and arrange appropriate care. However, medical and nursing staff in the ED are responsible for any emergency care necessary. This shared care is seamless and collaborative.

Managerial

The management responsibility of the post holder will be to the Clinical Director who is responsible to the Chief Executive and Trust Board.

Clinical Audit and Clinical Governance

The post holder must be aware of clinical governance and clinical risk management and take an active part in their implementation, including audit. Clinical audit is established at the BRI. They will participate in at least one supervised audit project during the 12 month post and will be encouraged to attend interdisciplinary clinical governance meetings

On-Call Commitment

In ED the roster is a full shift system. Shifts are a balanced mix of earlies, mids, lates, and nights. The post’s pay will be calculated by HR and will be compliant with the new regulations set in Aug 2017.
